The error which shows up is "Error creating file system: Cannot run mkfs: cannot spawn 'mkfs.btrfs -L "New Volume" /dev/dm-0': Failed to execute child process "mkfs.btrfs" (No such file or directory)"

Looks like you need to install btrfs-tools.
