kali should update that page http://docs.kali.org/downloading/kali-linux-live-usb-install#osx cause it's not working on lastest macOSX Sierra. Even the rREfind version is old...

Due to the age of the OS (Kali Moto [v1], Kali Safi [v2], Kali Rolling 2016.x), these legacy versions are no longer supported.
